Rett Syndrome (RS) is a neurological disorder seen almost exclusively in females, and found in a variety of racial and ethnic groups worldwide. 레트 증후군이란 거의 여성에게 나타나며, 전세계적으로 다양한 민족에게 발견되는 신경장애이다.
First described by Dr. Andreas Rett, RS received worldwide recognition following a paper by Dr. Bengt Hagberg and colleagues in 1983. 레트 증후군은 Andreas Rett 박사에 의해 처음으로 언급되었으며 1983년 Dr. Bengt Hagberg와 그의 동료들이 쓴 논문 이후에 세계적으로 알려졌다.
The child with RS usually shows an early period of apparently normal or near normal development until 6-18 months of life. A period of temporary stagnation or regression follows during which the child loses communication skills and purposeful use of the hands. Apraxia (dyspraxia), the inability to program the body to perform motor movements, is the most fundamental and severely handicapping aspect of RS. RS is most often misdiagnosed as autism, cerebral palsy or non-specific developmental delay. While many health professionals may not be familiar with RS, it is a relatively frequent cause of neurological dysfunction in females. Most researchers now agree that RS is a developmental disorder rather than a progressive, degenerative disorder as once thought. While there is strong evidence of a genetic basis, the origin and cause of RS remain unknown. Many girls begin independent walking within the normal age range, while others show significant delay or inability to walk independently. Some begin walking and lose this skill, while others continue to walk throughout life. Repetitive hand movements including one or more of the following: hand washing, hand wringing, hand clapping, hand mouthing, which can become almost constant while awake 다음의 한가지 또는 그 이상을 포함하는 반복적인 손 동작:( 손을 씻는 듯한 동작, 손을 비틀어 짜는 듯한 동작, 박수치기, 손을 입으로 가져가기)은 깨어있는 동안 거의 지속적이다.

Symptoms not required for the diagnosis, but which also may be seen. These features may not be observed in the young girl but may evolve with age. 진단에 필수적인 증상들은 아니지만 관찰되어질 수 있다. 이러한 특징들은 어린 소녀에게서 나타나지 않을 지 모르나 나이를 먹으면서 나타나기도 한다.
Breathing dysfunctions which include breath holding or apnea, hyperventilation and air swallowing which may result in abdominal bloating and distension 숨을 죽이거나 무호흡, 호흡항진(갑작스럽게 한꺼번에 내쉬는 형태) 그리고 공기빨아들이기를 포함하는 호흡장애는 복부를 부풀게하고 팽창시키는 결과를 가져올지도 모른다.
EEG abnormalities -- slowing of normal electrical patterns, the appearance of epileptiform patterns and loss of normal sleep characteristics EEG의 이상들-정상적인 전기 유형을 천천히 하게 할, 간질형태의 패턴의 출현과 정상적인 수면 특성의 상실

All girls and women with RS do not display all of these symptoms, and individual symptoms may vary in severity. A pediatric neurologist or developmental pediatrician should be consulted to confirm! the clinical diagnosis. 레츠를 앓는 모든 소녀와 여자들은 이러한 증상들을 모두 나타내지는 않으며 개인적인 증상들은 정도에 있어 다양하다. 임상 진단을 확신하기 위해 소아 신경과 의사나 발달 소아과 의사들에게 물어보아야 한다.
3. Stages(단계들)

This stage is usually overlooked, as the symptoms of RS are just emerging and are somewhat vague. 레트의 증상이 이제 막 나타나기 시작하였고 약간 모호하기 때문에 이 단계는 보통 간과되어진다.
The infant may show less eye contact and have reduced interest in toys. 영아는 눈맞춤이 줄고 장난감에 대한 관심이 줄어들 수도 있다.

Apraxia, motor problems and seizures are more prominent. 실행증, 운동 장애와 발작(경기)이 보다 두드러진다.
However, improvement is seen in behavior with less irritability and crying and less autistic features. This stage usually begins after age 10, and is characterized by reduced mobility. Some girls stop walking, while others have never walked. 이 단계는 일반적으로 10세 이후에 시작되며 운동성 감소로 특성지어진다. 몇몇 소녀들은 보행을 멈추고 나머지 소녀들은 결코 걸어본적이 없다.

4. Kids ask about Rett syndrome(아동들은 레트 증상에 관해 질문한다)
Most people have never heard of Rett syndrome. But you might know a girl with Rett syndrome! She may live in your neighborhood or attend your school. You probably noticed right away that she makes special movements with her hands almost all the time. Do you wonder why? Rett syndrome was named for Dr Andreas Rett, who first recognized the syndrome. A syndrome is a group of symptoms ? when you put them all together they identify a special condition. 4-2 What are the symptoms of Rett Syndrome?(레트의 증상들은 무엇인가?)
Rett syndrome almost always happens only in girls. When she is born, a girl with Rett syndrome seems to be healthy. She smiles, sits up and may begin to walk and talk. 4-3 What kind of problems does a girl with Rett syndrome have? 레트 소녀들은 어떤 문제들을 가지고 있나?
She has a lot of problems that affect her in different ways. But the biggest problem is apraxia. Because of apraxia, her body cannot do what her brain tells it to ? things like moving and talking. Just imagine how hard it would be to know what you want to do and not be able to do it, or to understand what someone says but not be able to show that you understand. Not being able to talk doesn’t mean she doesn’t have anything to say! She probably understands everything you say, but she cannot react the way you want her to. Pay attention to her body language and her eyes ? they tell you a lot! When she is irritable or loud, she is probably trying to tell you something important in the only way she can. A girl with Rett syndrome may need to take medicine to control seizures or to help her motor coordination or breathing. 레트 소녀들은 경기(발작)를 통제하거나 근육 협응과 호흡을 위해 약을 먹을 필요도 있다.

Physical therapy is used to help her keep flexible and able to move better and walk. Even girls with Rett syndrome are different from each other in some ways. Some girls go to special schools, while others attend their local neighborhood schools. They may learn in different ways, but they love to learn, just like you. 5. Communication 의사 소통
Most girls with RS show an intense desire to communicate through their eyes, gestures and body language. 대부분의 레트 소녀들은 눈, 몸짓 등을 통해 의사소통을 하려는 강렬한 욕구를 보여준다.
There is often a delay in response to stimuli. Possible alternative communication methods include eye-gaze response, picture, letter and word boards and touch or switch operated voice output devices. 6. Education(교육)

The more she has to think about the process, the more difficult it becomes. 7. General (일반적인 사항)
Scoliosis develops in a considerable number of girls with RS and may progress rapidly during ages 8-14. In some, bracing and/or surgery may be required. However, many girls do not show rapid progression. The likelihood of worsening scoliosis seems to be increased in girls who do not walk and those who have early low muscle tone or dystonia. Seizures are reported in a high number of girls with RS, but the incidence of true clinical seizures is felt to be overestimated. Many common RS motor behaviors are mistaken for seizures (shown during polygraphic monitoring). It is felt that many of these events are non-seizure, while actual seizures may be under-recognized and occur during sleep. Video-EEG monitoring may be necessary to determine appropriate treatment. Growth in RS is usually slowed, with most girls and women quite small for their age. Studies have shown that despite what appears to be voracious appetite, many girls meet the criteria for moderate to severe malnutrition. This may come about due to swallowing difficulties, inadequate intake of food, energy expenditure imbalance, or inadequate utilization of nutrients. Eval!uation for swallowing difficulties and gastroesophageal reflux is indicated for many girls with RS. Bone fractures are seen more often in RS than in other motor disorders. This is due to low bone-mineral content and decreased bone density, in spite of adequate calcium intake. Agitation is often thought to result from frustration over the inability to communicate. It is helpful to keep a detailed log including a list of the girl's behaviors and activities, in an attempt to determine and remedy factors that influence agitation. Physical causes for crying should always be considered. Once physical factors are ruled out, strategies such s warm baths, massage, music or a quiet, less stimulating environment can be helpful. Small frequent meals during the day with added carbohydrates may help. Constipation is common and can result in severe discomfort. Adequate fluid intake, high-fiber foods and exercise are important. It is preferable to avoid long-term use of laxatives, suppositories and enemas, but stool softeners, mineral oil and Milk of Magnesia are often helpful. 변비는 일반적이며 심한 불편함을 가져올 수 있다. 충분한 액체의 섭취, 고섬유질 음식과 운동이 중요하다. 설사약, 좌약, 관장약을 오랜 기간동안 사용하는 것을 피하는 것이 바람직하지만 장을 부드럽게 해 주는 것, 미네랄 오일, 산화 마그네슘의 우유가 종종 도움이 된다.
